The blue is the 12"x14" hole
that was cutout of the exhaust system duct. This would allow a fire to
escape the ductwork and then into the ceiling of the establishment.

This is over where people
live in an assisted living 6 story community.
Can you spot the deficiency?

The kitchen exhaust ductwork
is leaking grease into the surrounding area.
This makes the ductwork
unable to contain a fire in the worse case situation. Even if the duct is
wrapped in fire wrap, being soaked in grease hinders its ability.
